The Air Force Board for Correction of Military Records (AFBCMR) is the highest level of administrative review within the Department of the Air Force. Unless procured by fraud, a decision is final and conclusive on all officers of the United States.
Once applicable records are obtained, applications are screened to determine if they may be corrected without referral to the AFBCMR. In many situations, corrections to military records may be made administratively.
Records may be obtained and the request evaluated by the Office of Primary Responsibility (OPR). Applications that are not administratively corrected by the OPR will be considered by the AFBCMR. A copy of all advisory opinions are referred to the applicant or the applicant's counsel for response.
Except in those cases where a personal appearance is granted, the AFBCMR bases its decision on the evidence contained in the case file. This normally consists of military records, an advisory from the OPR, and statements, arguments, and documents provided by the applicant. The burden of proof of either error or injustice rests with the applicant.
Active duty and former members of the Regular Air Force, Air Force Reserve, and Air National Guard, to include Air Force Academy and Reserve Officer Training Corps (ROTC) can apply.
If the former member is deceased or incompetent, surviving spouse, former spouse, next of kin, or a legal representative may apply with supporting documentation, e.g., power of attorney, marriage license, death certificate.
- For a United States citizen born in the United States or one of its possessions, the original or certified copy (not a notarized copy) of birth certificate on file with the office of vital statistics of the state or United States possession in which the member was born is required.
- For a United States citizen born in a foreign country, a certificate or statement verifying the date and place of birth furnished by the United States Consular Office or the service hospital where the member was born is required.
- For a naturalized citizen, the original or certified copy of a certificate from the Bureau of Immigration verifying the date and place of birth is required.
- Active duty members can take their source documents to their local MPS to have the changes corrected administratively. There is no need to go through the Air Force Boards for the Corrections of Military Records process.
- Former Air Force members and Retirees must fill out a DD Form 149. Mail the DD Form 149 to the address listed on the back of the form, along with the source documents.
Note: The Air Force does not correctrecords of former members unless evidence proves the date of birth/place of birth used while serving with the Air Force was erroneously recorded.
Active Duty
Active duty members can take their source documents to their local MPF to have the changes corrected administratively. There is no need to go through the Air Force Boards for the Corrections of Military Records process.
Former Air Force Personnel
Former Air Force members and Retirees must fill out a DD Form 149. Mail the DD Form 149 to the address listed on the back of the form, along with the source documents.
Note: The Air Force does not correct records of former members unless evidence proves the citizenship used while serving with the Air Force was erroneously recorded.

Required Document
The original, a certified copy, or a notarized copy of the social security card.
If your contention involves a medical issue, you must also fill out a consent to release your medical records from the Veterans Administration (VA). You must fill out a:
